    Vehicle must be registered in Cudahy

    Cannot have more than 2 unpaid parking citations

    No more than 3 annual permits per address permitted 

31. Acknowledgement

    Submission of this form does not grant you an overnight parking permit for this vehicle. Documents must be approved by city staff. Once approved, you will be notified via email (check spam/junk folder) and the registered owner will be required to sign the application and make payment for the permit in person at City Hall. 

    Note: Parking without a permit is subject to a citation.
